SUPPLEMENTAL FORMS

In order to complete the application process, please complete the checklist items listed below, including submission of all supplemental application forms.

• Submit completed Online Application and application fee of $100.
• Submit a copy of Applicant's Birth Certificate.
• Forward Academic Release to student's current school.
• Forward Teacher Reference to student's current teacher.
• Forward Administrative Reference to student's current school.
• Read Health Information for the State of Hawaii.
• Submit copy of Custody Agreement at Parent Interview, if applicable.
• Schedule an appointment for Student Testing.
• Schedule a Family Interview.
